Reducing Human Error Overview
STAT-A-MATRIX offers a range of consulting and training services

Error Risk Reduction or ERR, is a strategic and methodological
approach to enhancing the reliability of human activities of organizations.
It is founded on the systematic identification and removal or
reduction of risk influencing factors (RIFs) which trigger or
amplify natural human vulnerabilities.
ERR can be applied to most kinds of organizations, small or large.
Changes needed to achieve opportunities for reliability improvement
tend not to be capital-intensive and seldom take long to implement.
Once the principles are understood, ERR is generally popular with
those most closely affected because it removes sources of frustration,
as well as gaining corporate benefits of cost and service improvement.
ERR concepts and methodologies are distinct from, and complementary
to, continuous improvement disciplines such Six-Sigma and Lean.
ERR proactively removes causes of errors, even those that have
yet to occur.

The Error Risk Survey (ERS) provides organizations with a powerful
means of gauging their vulnerability to human error and how effectively
they are addressing it. It asks those who carry out activities
some revealing questions about their work and observations. These
“tip of the iceberg” indicators can describe the current
situation, show improvements, and make comparisons within the
organization, or externally.
The ERS is provided free of charge to organizations that want
to know if they are doing enough to achieve reliable human performance
and gain competitive edge over those not addressing human error
effectively.
To use ERS, one coordinator from each organization needs to set
up a survey account and invite colleagues within the organization
to participate in the survey. The ERS system will help you set
up the survey account, invite participants, and track their progress.
Detailed results from the survey will be available to each participating
organization after several weeks.
